Transition to Post Secondary
Transition planning is a process that requires a partnership among the student, family, and school district and, as appropriate, other agencies that can provide transition activities to help students with disabilities move from school to adult life. Transition Services means a coordinated set of activities for a student with a disability designed to improve their academic and functional achievement to facilitate the student's movement from school to postschool activities. Transition planning and services are designed to prepare a student with a disability to achieve their post-secondary goals related to living, learning and earning within the community.

The Able-Ride / Paratransit Program provides public curb-to-curb transportation through the MTA-Long Island Bus service. Individuals with disabilities are evaluated and approved under federal standards contained in the Americans with Disabilities Act (ADA). The service is available only within Nassau County. Able-Ride will pick up the person with a physical disability outside the home and take the individual to visit the doctor, go shopping, commute to work, or even attend social events. Many people with disabilities are now holding productive jobs as a result of the Paratransit Program.
